Requirements

  • Master's Degree in Social Work is required
  • LMSW - Licensed Master Social Worker is required upon hire or a LCSW - Licensed Clinical Social Worker is required upon hire
  • CPR - Cardiopulmonary Resuscitation (BLS) is required upon hire
  • Knowledge of Microsoft Outlook and Office (Word, Excel)
  • Customer service skills
  • Strong oral and written communication skills
  • Strong commitment to interdisciplinary collaboration
  • Critical thinking, analysis and conflict resolution skills
  • Psychosocial and crisis intervention skills
  • Ability to prioritize and meet deadlines

Nice To Haves

  • 3 years of Social Work experience in a hospital/medical environment is highly preferred
  • Case Management / Social Work experience in a hospital setting is highly preferred
  • Case Manager / Social Work experience in a post-acute care facility helpful
  • 1 year of discharge planning/care management in a hospital setting is preferred
  • ACM - Accredited Case Manager OR CCM - Certified Case Manager is preferred upon hire
  • Other ANCC preferred upon hire
  • Strong knowledge/experience in maternal health, behavioral health, trauma care, and motivational interviewing.
  • Working knowledge of medical necessity criteria preferred
