About The Position

A Bachelor's prepared (BSW) Social Worker assists with securing safe discharge placement of patients through helping patients/families identify discharge needs, verify health benefits, provide discharge planning for appropriate level of care post discharge including facilitating placement/transfer to another facility and securing necessary medications for discharge. Maintains an accurate record of patient/family contacts and documents pertinent information in the medical record.

Requirements

  • Bachelor's degree in social work or related field.
  • 6 months of related experience.
  • Excellent organizational, interpersonal, and communication skills.
  • Knowledge of community resources.
  • Ability to utilize computer resources and have basic computer skills.

Responsibilities

  • Receives referrals and assesses in a timely manner those patients and families/significant others who are referred to social work service.
  • Integrates social work plan into overall patient care through interdisciplinary collaboration.
  • Provides social work services in the areas of comprehensive case management, discharge planning, and continuing care services.
  • Collaborates with the interdisciplinary team to develop discharge plans prospectively and conduct transitional planning for patients moving to nursing homes/other facilities.
  • Understands/utilizes hospital and community based resources/entitlements.
  • Refers patients, families/significant others, and hospital staff to appropriate services to insure continuity and quality of care.
  • Develops and utilizes specialized knowledge of resources related to the needs of specific patient populations.
  • Facilitates efforts to obtain insurance coverage for hospital and community based services.
  • Assists with assessing the patient's ability to secure prescribed medications at discharge and helps with access to pharmaceutical assistance programs/other medications resources as indicated.
  • Arranges for appropriate mode of transport for patients being discharge which can include ambulance service, verifying the medical certification form has been completed by the physician, arranging wheelchair van transport or coordinating family transport of the patient.
  • Documents the patient care plan, staff interventions, and outcomes promptly in the medical record.
