The Social Worker provides psychosocial/emotional assessment and therapeutic clinical counselling services to clients in assigned clinical areas, utilizing various modalities and adhering to professional standards. They participate in interdisciplinary team care planning, discharge planning, and resource linkage from a Social Work perspective. Additionally, they participate in committees, provide education and consultation to clients and the interdisciplinary team regarding psychosocial/emotional aspects of illness/injury/disease/health and Social Work practice.

Requirements

  • Bachelor's Degree of Social Work from an accredited school of Social Work.
  • One (1) year's recent, related experience.
  • Current full registration with the BC College of Social Workers.
  • Knowledge and understanding of psychosocial/emotional issues related to illness/injury/disease, and current Social Work treatment modalities.
  • Knowledge of counselling skills, including brief solution focused therapeutic (individual/group), crisis intervention, and resource counselling.
  • Ability to demonstrate a high level of knowledge, skill, effectiveness, self-awareness, and self-correction in verbal communications.
  • Knowledge of the key issues and current Social Work practice in geriatrics and chronic disease management.
  • Knowledge of applicable legislation/acts (AGL, Mental Health, Child Protection, FOI, Organ donation) and ability to apply their principles.
  • Ability to communicate effectively verbally and in writing with clients, the healthcare team, and community agency personnel.
  • Ability to work effectively with others, including in a multicultural environment, independently, and as part of a multidisciplinary team.
  • Ability to educate and provide consultation to clients and healthcare professionals.
  • Ability to participate with the interdisciplinary team in care planning for effective utilization of client/system resources.
  • Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills.
  • Ability to organize and prioritize workload in a complex environment.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.

Responsibilities

  • Conducts psychosocial/emotional assessments of clients using methods such as focused interviews, data collection, and interpretation within the client system context, adhering to professional standards and clinical policies.
  • Provides basic therapeutic clinical counselling services based on psychosocial assessments, identifying client/system strengths and challenges, and intervening to improve psychosocial/emotional wellbeing with a defined plan of care.
  • Educates and consults with clients and healthcare professionals on Social Work practice and clinical specialty areas, including adaptation to illness/disability, trauma, bereavement, critical incident stress management, relevant legislation, chemical dependency, and community support services.
  • Maintains client records by documenting assessments, charting information, and preparing progress notes and reports according to established standards.
  • Provides input to the interdisciplinary team in client care planning and discharge planning by assessing client needs, articulating analysis, participating in team meetings, and collaborating with clients and community partners.
  • Communicates with community health and social agencies, including government ministries, Homehealth personnel, and family physicians, by collaborating on risk assessments, making referrals, and exchanging information.
  • Acts as a resource to clients by providing information on community resources, facilitating referrals, and developing community resources to support family health.
  • Participates in ongoing quality improvement by engaging in program/practice evaluation, identifying opportunities for change, assessing resources/gaps, initiating solutions, and analyzing outcomes.
  • Maintains a variety of records, such as workload statistics, by recording information, submitting data, and providing input on data trends and improvements.
  • Maintains professional development by reviewing literature, consulting with experts, evaluating clinical practice, and participating in professional development activities.
  • Performs other related duties as assigned.
