About The Position

In this role, you'll play an important part in supporting the emotional and social well-being of skilled nursing residents by ensuring their individual needs are understood and addressed. You'll build meaningful relationships, collaborate with healthcare professionals, and help deliver person-centered care that promotes comfort, dignity, and quality of life.

Requirements

  • Bachelor's degree from an accredited School of Social Work OR a bachelor's degree in a related human services field, such as Social Work, Sociology, Psychology, Rehabilitation Counseling, Special Education, or a closely related discipline.
  • One (1) year of supervised social work experience in a healthcare setting working directly with individuals.
  • Experience working with geriatric, skilled nursing, long-term care, or rehabilitation populations.
  • Strong communication, advocacy, care coordination, and problem-solving skills.

Responsibilities

  • Conduct psychosocial assessments and develop individualized care plans that support residents' emotional, social, and overall well-being.
  • Partner with residents, families, and interdisciplinary care teams to identify needs, establish goals, and coordinate services.
  • Provide guidance, counseling, advocacy, and support to residents and their families throughout their healthcare journey.
  • Connect residents with community resources, financial assistance programs, and specialized services to enhance quality of life.
  • Coordinate discharge planning and care transitions to ensure a smooth and successful move to the next level of care.
  • Advocate for residents' rights, preferences, and psychosocial needs while promoting dignity and person-centered care.
  • Maintain accurate documentation and ensure compliance with federal, state, and local regulations, as well as organizational policies.
  • Participate in quality improvement initiatives, team meetings, training programs, and collaborative efforts that support exceptional resident care.
