About The Position
The social worker will primarily provide services to 7 West inpatients. The services provided range from psycho-social assessment and treatment; crisis intervention; advocacy; supportive counseling; consultation; discharge planning; palliative care; patient/family/staff education; liaising with community resources. The Social Worker will enjoy a challenging practice, which requires excellent clinical skills, flexibility and a desire to work both independently and collaboratively in a variety of settings.
Requirements
- Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
- Masters degree in Social Work or Baccalaureate degree in Social Work from an accredited School of Social Work
- Knowledge of Healthcare Consent Act, Substitute Decisions Act, Child and Family Services Act
- Excellent knowledge of community support and placement services
- Excellent crisis intervention skills
- Proven ability to work as a member of interdisciplinary healthcare teams
- Proven ability to organize and prioritize patient caseload across several programs
- Excellent communication skills
