About The Position
Avita Health System is seeking a dedicated Social Worker BSW to join their Case Management Department at their hospital locations. The role involves assessing patients and families, providing intervention and treatment to support patients, addressing psychosocial needs and adjustment patterns throughout hospitalization, and collaborating with a multidisciplinary team to create treatment plans that maximize emotional, social, and physical well-being. The Social Worker will also provide information and education regarding healthcare and community resources.
Requirements
- Bachelor’s degree.
- Current State of Ohio Social Work license.
Responsibilities
- Assess patients and families and provides intervention and treatment to support patients.
- Address psychosocial needs and patterns of adjustment throughout the hospitalization.
- Collaborate with the multidisciplinary team to formulate treatment plans to maximize emotional, social and physical well-being.
- Provide information and education regarding health care and community resources.
