About The Position

Full-Time, 30 hours per week Home Health JOB SUMMARY Responsible for the delivery of social services to high-risk patients when admitted to Home Health. Services may include advocating residents’ rights, supportive counseling, and providing needed information concerning living units, Medicare, Medicaid, community resources and discharge planning. Offers assistance to patients and their families in the form of various support services. Accountable for compliance with State/Federal laws and regulations. Supportive of the Village’s mission, values, and service/hospitality (PRIDE) expectations. Completes training as required by policy. ESSENTIAL JOB FUNCTIONS Uses professional judgment for initial and ongoing assessment, planning, and evaluation of psychosocial, economic, and environmental needs of patients and families. Contacts patients and/or families for initial assessment and evaluation of social work needs Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect. Assesses environmental resources and obstacles to maintaining safety, including the caregiver’s ability to function adequately. Assists patient/family with utilizing community resources. Submits paperwork in a timely manner to update and maintain patient medical records. Assessment of the social and emotional factors related to the patient’s illness, the need for care, their response to treatment, and adjustment to care. Assessment of the relationship of the medical and nursing requirements to the home situation, financial resources, and the community resources available Performs crisis intervention with problems such as alcoholism, death, depression, anxiety, and loneliness. Participates in case conference for all patients and identifies patients that could benefit from social work assessment. Plans for coordinated Psychosocial support with other disciplines or community resources; consults with others as necessary. Works with patients in a counseling situation to help meet their psychosocial needs. Keeps abreast of current legislation, trends, resources, and facilities related to the field and fulfills all requirements set forth by Federal/State regulations and laws. Adheres to all safety regulations and requirements. Handles stress well and gets along with others within the workplace and with customers outside the department. Maintains acceptable standards of workplace conduct. The responsibilities described above represent the primary responsibilities of the job. Other responsibilities may be assigned by the supervisor as warranted by business needs. The incumbent is expected to perform all assigned responsibilities.

Requirements

  • Ability to determine time, place and sequence of operations or action to be taken on the basis of analysis of data; executing determinations and/or reporting on events.
  • Ability to deal with individuals in terms of their total personality in order to advise, counsel, and/or guide them with regard to problems that may be resolved by legal, scientific, clinical, spiritual, and/or other professional principles.
  • Ability to use body members, hand tools, and/or special devices to work, move or carry objects or materials. Involves little or no latitude for judgment with regard to attainment of standards or in selecting appropriate tool, object, or materials.
  • Ability to apply principles of logical or scientific thinking to define problems, collect data, establish facts, and draw valid conclusions. Interpret an extensive variety of technical instructions in mathematical or diagrammatic form. Deal with several abstract and concrete variables.
  • Ability to compute discount, interest, profit and loss, ratios, and percentages. Calculate variables and formulas, ratio, and proportion variables.
  • Ability to read literature, books, technical journals, and legal documents. Write journals, reports, and correspondence. Conversant in persuasive speaking, discussions, and debates.
  • Ability to communicate with an interdisciplinary team and advocate for the needs of the residents.
  • Education: BSW degree in Social Work or Masters in Gerontology required.
  • Licenses/Certifications: Valid driver’s license and proof of insurance required.
  • Licensed BSW in Missouri
  • Licensed BSW in Kansas; or ability to obtain within 90 days of hire.
  • Knowledge and Skills (required unless otherwise noted): Basic level knowledge of Microsoft operating system and Microsoft Word, Excel, and Outlook.
  • Ability to manage multiple tasks and responsibilities at the same time.
  • Must be able to work autonomously.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals; calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions, and deal with a variety of abstract and concrete variables.
