About The Position

Responsible to plan, organize and implement social work service to patients and families. Maintains contact with referring agencies and related people, and with community resources used for referral. The Social Worker Associate will complete psychosocial evaluations, and in collaboration with the interdisciplinary treatment team, will be responsible for discharge planning and aftercare of patients on the unit. May assist with direct patient care in various units.

Requirements

  • Social Work license in the state of SD Upon Hire.
  • Bachelor's Social Worker Associate - Licensing Board active in state of practice Upon Hire
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others
  • Ability to hear, understand and distinguish speech and other sounds.

Responsibilities

  • Interviews patient and family members to obtain information about home environment, family relationships, health history, and patient’s personality traits.
  • Evaluates data gathered in terms of the medical plan of treatment, available social service programs, and plan a pertinent therapy program that will provide maximum benefits for each patient.
  • Assists patients and families to understand, accept, and follow medical recommendations.
  • Performs a variety of services such as advising on social problems, arranging for discharge care at home or in institutions, placement of patients into foster homes or temporary placement, and alleviation of anxieties and fears.
  • Utilizes resources to assist patient to resume life in the community or to learn to live with a disability.
  • Prepares required documentation of each case.
  • Maintain records, reports and other duties as required.
  • Maintains department policies, procedures, objectives, and performance improvement program, safety, infection and environmental control.
  • Serves as the social work representative on multi-disciplinary team approach to patient care.
  • Provide community education services and group activity service.
