About The Position

The University of Rochester is seeking a Social Worker Assistant to join their Psychiatry SMH Social Work department. This full-time position involves assisting unit social workers in providing services to patients and their families admitted for mental health treatment. The role is integral to the interdisciplinary treatment team, contributing to the development and implementation of patient treatment and discharge plans. A key responsibility is coordinating the discharge planning process for patients on Adult Inpatient Psychiatry and Child and Adolescent Inpatient Psychiatry units. The Social Worker Assistant addresses psychosocial problems that may hinder care or discharge, develops interagency relationships, and mobilizes community resources. Additionally, the role includes providing follow-up telephonic support to post-discharge patients for two weeks to address identified barriers.

Requirements

• Bachelor's degree in Social Work required
• Or equivalent combination of education and experience

Responsibilities

• Rounds on identified Adult inpatient psychiatry patients prior to their discharge date/time for a warm handoff prior to their discharge from the unit for review of discharge plan via teach back method.
• Reaches out to discharged patients from Adult Inpatient Psychiatry, telephonically for two weeks post discharge to address any barriers identified in the discharge plan.
• Assists the Unit Social Worker in the coordination of the patient’s identified discharge plan. This may include referrals, continuity of care, safety planning and/or assistance with the completion of other necessary discharge documentation.
• Documents interventions in compliance with Division, Department and Hospital Policy, to include any safety plans or other documentation crucial to discharge planning and continuity of care.
• Attends Social Work Division and Department Staff meetings, as required. Participates in activities of the Social Work Division including but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.
• Subscribes to the Departmental goal of creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competence events and training appropriate to job duties.
• May provide social work assistant coverage and back-up as needed during the absence of co-workers in the inpatient services.
• Other duties, as requested and assigned.
