About The Position

The social work assistant functions as a Targeted Case Manager (TCM) and is primarily responsible for case management services across Strong Ties Clinic. As part of a Certified Community Behavioral Health Clinic (CCBHC), TCM services will assist people receiving services in sustaining recovery and gaining access to needed medical, social, legal, educational, housing, vocational, and other services and support. CCBHC TCM provides an intensive level of support and is intended for individuals with complex or serious mental health or substance use conditions, or for those who have a short-term need for support during critical periods, such as an acute episode or care transition. CCBHC TCM includes support for people deemed at high risk due to health-related social needs, suicide or overdose, and particularly during times of transitions such as from residential treatment, hospital emergency department, incarceration, or psychiatric hospitalization. Under general direction and with considerable independence, the TCM performs several care management activities consistent with CCBHC and the Department of Psychiatry Policy. They establish and maintain cooperative working relationships with community providers and resources to obtain needed services and support for enrolled patients. The TCM works as part of an interdisciplinary team to support activities to engage patients in care, connection to care management, and other resources, ensuring necessities are met.
