About The Position
Juniper Communities is seeking a compassionate and caring Social Worker Assistant for their Spearly location. This is a full-time position with a Tuesday to Saturday schedule and rotational on-call duties. The facility specializes in mental and behavioral health for residents with conditions such as schizophrenia, bipolar disorder, TBI, failed suicide attempts, and more, in addition to typical Alzheimer's/dementia diagnoses. The ideal candidate will be caring, value-driven, and able to assist residents in need.
Requirements
- 1 year of relevant experience.
- Knowledge of Medicaid/Medicare, VA, hospice, ULTC's, Social Security, Title 19, MCD redetermination and application process, ancillary scheduling and facilitation, rep payee.
- Ability to pass a background check and drug test.
- Caring and value-driven attitude.
Responsibilities
- Assist residents in need.
- Provide social work support within a skilled nursing facility specializing in mental and behavioral health.
- Manage ancillary scheduling and facilitation.
- Handle rep payee responsibilities.
- Assist with Medicaid/Medicare applications and redeterminations.
- Facilitate hospice care.
- Understand and navigate VA benefits and Title 19 processes.
