Farmington Health Services (AKA Trinity Campus) is a five-star rated senior living community. Trinity Care Center offers 24/7 skilled nursing care and rehabilitation services. This is a new position that FHS plans to fill in July/August 2026. - Completing resident assessments
- Overseeing resident discharge process
- Addressing resident and family concerns
- Leading resident care conferences
