This permanent, full-time Social Worker position is within the Assessment and Reintegration Unit. The role involves care coordination, professional practice, community liaison, and program staff collaboration. The Social Worker will utilize evidence-based models to foster continuity of treatment, coordinate patient needs with community supports, and advocate for patient resources. Responsibilities include psychosocial assessments, providing recovery-based treatment, and coordinating treatment plans with patients, natural supports, social services, and community providers. The role also involves offering educational consultations, engaging in professional practice activities, and potentially providing supervision to social work students. As a community liaison, the Social Worker will represent the organization on various boards and committees, identify community program needs, and provide information to community agencies. Program staff duties include participating in hospital and program committees, engaging in ongoing education, and ensuring patient and organizational safety standards are met. This includes recognizing and reporting safety issues, implementing preventative actions, and complying with all relevant legislation and hospital policies. The role also contributes to continuous quality improvement and evaluation of clinical team services.
