About The Position

The Social Worker supports clients through a medically supervised withdrawal management program and healing programs. The role functions within the acute medical team and works in cooperation with interdisciplinary team members to deliver client care in accordance with established policies, standards, and procedures.

Requirements

  • Bachelor’s Degree in Social Work
  • Qualifies to register with the governing body (i.e. Registered Social Workers)

Nice To Haves

  • Previous experience in addictions treatment, addictions counselling, advocacy, community mental health, etc.

Responsibilities

  • Foster and nurture trusting relationships with clients, families, and communities through culturally safe and trauma-informed engagement that respects individual experiences and promotes healing.
  • Complete needs-based assessment to identify client strengths, immediate needs, risks, supports, barriers to recovery, and social determinants of health.
  • Conduct suicide risk assessments, crisis intervention, and safety planning in collaboration with the interdisciplinary care team.
  • Develop individualized discharge and transition plans that support continuity of care and successful movement into treatment programs, community services, and recovery supports.
