This position serves as manager for skilled and intermediate care for Medicare veterans.
Requirements
- Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW)
- A master's degree in social work
- Four years of social services experience.
Responsibilities
- Supervise Case Manager for medically complex, intermediate and skilled care including Medicare veteran units with a caseload of veterans with multiple psychiatric, substance abuse, behavioral and medical problems.
- Resident care in a 156-bed long-term care facility.
- Assure Case Managers escort and assist veterans to medical and other appointments.
- Responsible for admissions protocol involving financial affairs, medical records and placement on their assigned units and manage other Social Service staff.
- Investigates and reports resident complaints and incidents, recommending corrective actions and collaborating with Supervisor.
- Assure Case Managers updates veterans face sheet, represents department ay monthly Residents meetings.
