The Social Worker 2 role within the University Hospitals Lake West Medical Center offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists individuals with various issues. We currently offer an opportunity for a PRN Social Worker to support patients in Willoughby. Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital. Previous work in a hospital setting is strongly preferred. This PRN position will travel between Lake West and TriPoint Medical Centers. The Social Worker will be required to work in any unit of the hospital as needed, to perform psychosocial assessments, provide support to patients and families, and assist with discharge planning. It is needed to cover weekdays and weekends.

Requirements

  • Master's Degree from an accredited school of Social Work (Required)
  • Licensed Social Worker (LSW) (Required)
  • Computer skills (Required proficiency)
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)
  • Works autonomously (Required proficiency)
  • Communicates effectively with persons of various backgrounds (Required proficiency)

Nice To Haves

  • Hospital/Health Agency experience.
  • Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility)
  • Previous work in a hospital setting is strongly preferred.

Responsibilities

  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Provides age specific care to patients/families in all age groups.
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals
  • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.
  • Performs other duties as assigned.
  • Complies with all policies and standards.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
