DESCRIPTION OF WORK This role provides social services that support residents and their families during times of change or need. You will guide residents through assessments, care planning, and support programs. As a Social Worker 2, you will perform the following duties: Care Plan Participation: Attend care plan meetings and maintain complete documentation of social service activity Behavior Support: Provide behavioral and psychosocial support, as well as develop strategies to address emotional or behavioral needs Rights and Advocacy: Inform residents about their rights and assist with advanced directives, grievances, and voting status interviews Counseling Services: Provide individual and group counseling using methods such as supportive counseling, crisis intervention, or behavior modification Discharge Coordination: Plan resident discharge or transfers by arranging follow-up services, transportation, and any related needs Interested in learning more? Requirements

  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) (must have been employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment)
  • Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor’s degree in social work or social welfare
  • Two years of professional experience providing direct public or private social work services, and a bachelor’s degree in social work or social welfare
  • A master's degree in social work or social welfare
  • Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree.
  • All employees in the Department of Military and Veterans Affairs require an active license as a Social Worker or Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.
  • You must meet the PA residency requirement.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require a medical examination.
  • This position falls under the provisions of the Older Adult Protective Services Act.
  • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
