Requirements

• A bachelor's or master’s degree in social work or social welfare.
• Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
• You must meet the PA residency requirement.
• You must be able to perform essential job functions.
• A conditional offer of employment will require a medical examination.
• A conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

Responsibilities

• Conduct psychosocial assessments, attend team meetings, and maintain complete documentation that reflects social service activities
• Complete resident assessment instruments and behavioral evaluations and integrate findings into person-centered care plans
• Provide social work services for behavioral and emotional needs, including counseling and behavior management techniques
• Offer counseling and education to residents and family members to support improved personal and social functioning
• Evaluate readiness for discharge and coordinate plans, transportation, and summaries of needs with family and care teams
• Follow safety rules, report hazards, participate in required training, and support quality improvement efforts
