Requirements

  • Master’s degree in social work from an accredited school of social work.
  • Bachelor’s degree in social work and one year of experience in the area of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, or treatment planning.
  • Bachelor's degree in a human service field with at least 25 semester hours of approved coursework and two years of experience in a related area
  • Bachelor's degree and three years of related experience.
  • A valid driver's license is required.
  • The applicant selected must undergo a criminal background check and pass a drug screening test prior to employment.

Responsibilities

  • Coordinate/manage daily activities of up to four (4) Nutrition Social Worker II's and up to nine (9) food site monitor and delivery driver staff.
  • Make case assignments, set work objectives, review case documentation, review service plans, quarterly assessments, annual assessments, service provision authorizations, and hold regular conferences with staff to monitor Social Workers' performance.
  • Plan, organize, and direct all program staff in providing nutrition services to older adults.
  • Review nutrition caseload and make decisions for services needed.
  • Coordinate the operation of the Adult Nutrition Program by negotiating and monitoring contracts with caterers and building owners of meal sites (eight) assuring compliance with state, county, and agency policies and procedures.
  • Assure completeness of program files and records, preparing monthly and quarterly reports and maintain working knowledge of program budget to assure expenditures are within set budget and service goal needs to generate revenue.
  • Set service objectives, participate in the annual budget process, and analyze needs of the community to determine future needs of the program.
  • Provides direction and counsel for problem situations encountered by social workers by jointly processing case information, giving ongoing supervisory support, seeking assistance from Division Coordinator/Administrator as necessary.
  • Monitor and evaluate the services provided by reviewing case records for program compliance and provision of services.
  • Provide oversight and management of program special projects.
  • Manages unit personnel by conducting interviews, making hiring recommendations and training new staff as needed.
  • Provides ongoing training to staff according to program changes and/or staff needs and by making performance evaluation, promotion and disciplinary recommendations.
  • Develop volunteer support for the HDM component by recruiting organizations to house the meal program in each Gaston County township and ensuring that the program has the volunteers necessary to function, totaling nearly 850 volunteers.
  • Promotes Adult and Aging Services in the community by speaking to community groups, participating in health fairs and other educational/collaborative sessions.
  • Participate in other collaborative efforts as appropriate.
  • Reports necessary information by maintaining client database and compiling statistical information in order to analyze intake contacts/referrals/reports/ancillary programs and other data as needed.
  • Fully implement ServTracker components to track meal delivery, volunteer schedules and route coverage, and automated route development and optimization.
  • Must be available to work mandatory emergency shelters & other duties as assigned
