About The Position

The Ohio Veterans Homes, part of the Ohio Department of Veterans Services, are trauma-informed care long-term care facilities focused on providing excellent care to Ohio’s Veterans. The Veteran Homes provide direct nursing home care at facilities in Sandusky and Georgetown as well as domiciliary living to qualified veterans in Sandusky. We aim to maintain an inclusive work environment and commit ourselves to develop talented, inclusive, and dedicated employees. This position is responsible for directing Social Work Case Management in the Nursing Home, planning, developing, and supervising the Nursing Home Social Services Program for Nursing Home residents. The role involves conducting initial orientation of residents and their families regarding Ohio Veterans Homes policies, procedures, and programs, as well as assigning and monitoring cases, evaluating work performed, adjusting grievances, orienting, training, and counseling employees. The supervisor will attend training sessions and workshops to provide input into the Plan-of-Care and discharge planning. They will also supervise Nursing Home social workers who prepare and update social histories and all required paperwork relative to services provided. Additionally, the supervisor directs meetings of the Resident's Council and establishes the Resident's Grievance Committee as mandated by Ohio Revised Code, providing minutes of all meetings to the Nursing Home Administrator.

Requirements

  • Requires current license as independent social worker (i.e., LISW) as issued by State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board in accordance with ORC 4757.27
  • 12 months of experience providing clinical supervision
  • Pursuant to ORC 4757.41 (A)(5) any person engaging in social work or professional counseling as a civil service employee as defined in section 124.01 of the Revised Code who has at least two years of service as of July 10, 2014 is exempt from current licensure as independent social worker (i.e. LISW) requirement.
