About The Position

The Social Work Specialist works in collaboration or by delegation from CSWA or LCSW to screen patient/family for psycho-social needs and determines current and anticipated patient care concerns. The SW Specialists advocate for patients and families to connect with community resources. The SW Specialist provides families with information regarding patient care issues; works with OHSU, patients and families regarding guardianship and conservatorship issues, facilitates in the coordination of follow up care in OHSU clinics or in the community; participates in patient and family health care education, assists with housing and transportation needs as necessary. The Social Work Specialist provides specific service line support for patients in the inpatient units Function/Duties of Position The Social Work specialist screens patient/family for psycho-social needs and determines current and anticipated patient care concerns. The SW Specialist advocates for patients and families, screens for abuse, suicide or medical neglect and makes appropriate referrals. The SW Specialist screens patients and families’ ability to cope with major life changes, provides support to patients and families, and refers to a Medical Social Worker as necessary. The SW Specialist provides families with information regarding patient care issues; works with OHSU, patients and families regarding guardianship and conservatorship issues, facilitates in the coordination of follow up care in OHSU clinics or in the community; participates in patient and family’s health care education, assists with housing and transportation needs as necessary. Remote work at OHSU is a work arrangement under which an employee performs the duties and responsibilities of their position, and other authorized activities, from a non-OHSU location as part of their approved, regular work schedule. The Social Work Specialist generally is expected to be in person at least half of their work time, depending on the expectations of their specific position. The Social Work Specialist should have a private space at home, dedicated to work, and no Protected Health Information should be printed. Documentation: Maintains accurate and timely documentation of patient care services, including charting, reports and documentation. Submits patient care statistics in a timely manner. Information Systems: Creates and assists in form development as needed. utilizes database access system for information relevant to patient’s care.

Requirements

  • Two years of social service work experience providing supportive counseling to clients on a one-to-one basis which includes full responsibility for handling each case plan; AND
  • A Bachelor’s degree, or three more years of experience which shows: a) knowledge of casework methods and techniques; b) knowledge of patient’s family’s rights; c) knowledge of medical terminology; and d)knowledge of implications of the effect of illness, injury and/or disability on patients and families.

Nice To Haves

  • 3 or more years of hospital experience
