About The Position

This is a professional social services position for the Medical Foster Care (MFC) program reporting to the Registered Nursing Supervisor or Community Health Nursing Director position. The individual in this position utilizes the social work process to provide pediatric and family social work expertise in the assessment, training, implementation, and evaluation of MFC program services in the greater Jacksonville area. Service area includes Baker, Clay, Duval, Nassau and St. John's Counties.

Requirements

• Ability to work with families and children with special health care needs.

• Ability to collect and organize data.
• Ability to access information from computer data systems and websites.
• Ability to use a computer for word processing, spreadsheets, and database applications, as well as for attending virtual meetings and training.
• Valid Driver’s License
• Willingness to complete home visits and travel up to 30% of scheduled hours

Nice To Haves

• Bachelor’s degree or higher in the social services or related field
• 1-3 Years of Child welfare experience and medical setting experience
• Working knowledge of Florida Medicaid as it applies to children and youth, and Department of Children and Families as it relates to foster care.
• Experience in the use of technology, electronic health records including computer systems/applications (e.g. Microsoft Office: Word, Excel, Outlook, Teams).
• Working knowledge of medical terminology as it relates to medical conditions, diagnosis and treatment plans.

Responsibilities

• Manage programmatic referrals, assist and complete admissions to MFC homes and discharge from program
• Schedule and participate in monthly assessments, staffing’s, and parent-child specific training
• Ensure compliance with program standards, policies, and in-home clinical records through quarterly quality assurance activities
• Complete annual program reviews of medical foster homes and implement corrective action plans and/or technical assistance.
• Provide ongoing training and educational opportunities for medical foster parents
• Partner with the local Community Based Care Agency to coordinate MFC admissions, discharges and any needed transition of care.
• Recruit, train, and evaluate medical foster parents utilizing Family Match and other available computer based systems.
• Act as MFC program liaison in communication with other community providers and stakeholders.
• Serve as a liaison between the MFC providers and the MMA plans.
• Maintain accurate records and electronic data for each child and parent
• Prepare and submit reports and manage program data as assigned.
• Complete home visits with either medical foster parents or birth parents with frequency as indicated.
• Attend and participate in meetings and community outreach as assigned.
