Social Work Manager - Care Coordination
About The Position
The Social Work Manager provides leadership and management to the Social Work staff within the Care Coordination Department. This job sets standards for practice excellence through individual and group supervision and by self-example. The Social Work Manager provides quality customer service to patients of all ages, their families, visitors, medical staff, clinicians and co-workers, ensuring that everyone will be treated courteously, quickly and with respect. This position also collaborates with other healthcare providers and community resources to provide cost-effective health care services across-the-continuum.
Requirements
- Licensed Clinical Social Worker (LCSW) in Virginia
- Current LCSW
- Minimum of seven (7) years of work experience in a medical setting
- Two (2) years in a lead or supervisory role
- Previous work experience identifying and utilizing community resources
- Masters Degree in Social Work from a Council on Social Work Education (CSWE) accredited program
- Lifting 20-50 lbs.
- Prolonged standing

- Must be able to conduct duties and problem solve independently.
- Supervise over 30 Social Workers.
- Represents the Department within the organization, university and community.
- Must be available (or able to delegate) to staff for consultation 24/7.
