About The Position
This position provides social work services and discharge planning for individuals at ESH. Develop and implement person-centered treatment and discharge plans in collaboration with seriously mentally ill consumers and other members of the interdisciplinary treatment team. Serve as a link between community agencies and others that support consumer’s recovery and return to community.
Requirements
- Minimum of a Bachelor’s in Social Work (BSW) is required.
- Experience providing case management and discharge planning services to the mentally ill population.
- Valid driver's license.
