This position provides continuity of care to inmates under the supervision of licensed professionals and in compliance with behavioral health guidelines. The role involves working with identified inmates to complete Medical Assisted Treatment intake packets, scheduling in-reach appointments with community MAT providers, and participating in these appointments, documenting them appropriately. The Social Work/Counselor I will collaborate closely with CDOC Community Behavioral Health, CDOC MAT Providers, Facility Case Managers, Community MAT Providers, and other community agencies. Additionally, the position will provide behavioral health community transition services to inmates releasing from CDOC, identify inmates participating in or eligible for MAT services, and provide psycho-education on topics such as MAT and community resources under the supervision of a licensed professional. A key responsibility includes maintaining a database and tracking inmates in the MAT in-reach service, monitoring inmate treatment and documentation for compliance with standards, and ensuring continuity of care. The position may also require providing coverage for other state facilities as needed.
