About The Position

Responsible for providing support for the social work case management services provided to the participants of the Senior TLC program. Provides social services support such as participant screening, resource acquisition, and referrals. Links participants with appropriate community resources as directed by social workers.

Requirements

  • Minimum of 1 year experience working with the frail elderly population.
  • Must be able to effectively work in a team environment.
  • Must possess excellent communication, interpersonal and conflict resolution skills.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Nice To Haves

  • Bachelor's degree in human services-related field preferred but not required.

Responsibilities

  • Place phone calls to participants and/or caregivers to obtain follow-up information for expressed interest in case management services (i.e., SNAP benefits applications, financial assistance with utility bills, housing applications, etc.)
  • Responsible for submission of applications (via e-mail, fax, or physically submitting in-person)
  • Assists participants with completing housing applications (including serving as a liaison between family/caregivers who can assist with applications) and maintains communication between property management representatives and participants.
  • Assists participants with food insecurities by retrieving food items as needed from community partners or by initiating Healthy Savings Benefit Cards
  • Assists with completion of applications for clothing closets, and coordinates pick-up of clothing items and distribution to participants.
  • Coordinates account set-up services for ‘My Social Security Online’ to obtain hard copies of benefit verification letters and maintains hard copy records of participants’ social security cards, identifications cards/driver’s licenses, and Social Security Administration Benefit Verification Letters.
  • Obtains STLC staff signatures for bereavement cards of deceased participants and mails them out or hand delivers them in-person.
  • Assists with completing applications for vehicle placards and identification cards through North Carolina Department of Motor Vehicles.
  • Provides communication between Primary Care Provider (PCP) and participants/caregivers for FMLA forms and Social Security Administration forms (i.e., Physician’s Medical Officer’s Statement of Patient’s Capability to Manage Benefits Form or Physician’s Medical Statement).
  • Supports the Senior TLC Social Work team in researching community resources: including the continued development of the community resource guide for participants (i.e., food, clothing, senior apartments, financial assistance, etc.), and a routine review of the current community resource documents/services to ensure they are kept up to date (i.e., housing applications, etc.).
  • Assists the Community Support Liaison as needed and serves as the Community Support Liaison when the liaison is out of the office.
  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
  • Other duties as assigned.
