Baptist Health is seeking a Social Work Case Manager to intervene with patients and families regarding psychosocial needs, assistance, and support in coping with chronic, acute, or terminal illness. Services include consultation, discharge planning, education, counseling, advocacy, and information referrals to address emotional, social, and financial aspects of care. The role involves providing crisis intervention, coordinating discharge plans, and acting as a liaison between patients and community resources. The Case Manager will identify patient and family psychosocial and environmental needs, collaborate with the interdisciplinary team on discharge planning, and make appropriate referrals to community services. They will also provide inpatient emotional support, counseling, crisis intervention, and bereavement services, and assess/intervene in situations of child or elder abuse/neglect and domestic violence. This position provides care to all patient groups: neonatal, infant, pediatric, adolescent, and adults.
