About The Position

Bartlett Regional Hospital is seeking a Social Work Case Manager to join their Case Management team. This is a casual position with pay determined by experience and the specific level (SWCM 1, SWCM 2, or SWCM 3). Casual employees receive a $2.00 per hour differential in lieu of fringe benefits. The role involves providing social work and discharge planning services, utilizing established policies, procedures, ACMA guidelines, and the NASW code of ethics. Responsibilities vary by level, ranging from trainee/entry-level with direct supervision to advanced-level practice with limited supervision, including behavioral health assessments and interventions. Documentation must meet Bartlett Hospital and CMS standards, and collaboration with partners is essential.

Requirements

  • SWCM 1: Three (3) years of postsecondary education within a social work program. Must graduate from an accredited social work program within one (1) year of hire.
  • SWCM 1: Must obtain Alaska Social Work License within one year of graduation from an accredited social work program.
  • SWCM 1: Workplace Violence Prevention Training within 90 days of hire.
  • SWCM 2: Graduate of an accredited social work program.
  • SWCM 2: Completes annual educational requirements.
  • SWCM 2: One (1) year Of experience working as a social worker in the area of health care, behavioral health, or substance abuse treatment.
  • SWCM 2: Current State of Alaska Social Work License (LCSW).
  • SWCM 2: Valid AK Driver's License required if assigned to perform patient transport duties.
  • SWCM 2: Workplace Violence Prevention Training within 90 days of hire.
  • SWCM 3: Master's Degree in Social Work.
  • SWCM 3: Two (2) years of experience working as a social worker in the area of healthcare, behavioral health, or substance abuse treatment.
  • SWCM 3: Current State of Alaska Social Work License (LCSW).
  • SWCM 3: Valid AK Driver's License required if assigned to perform patient transport duties.
  • SWCM 3: Workplace Violence Prevention Training within 90 days of hire.
  • All new employees are required to produce a negative drug screen for all illegal substances in the State of Alaska prior to the start of employment.

Responsibilities

  • Provide social work and discharge planning in predictable situations using established policies, procedures, ACMA guidelines, and NASW code of ethics.
  • Plan and implement complex individualized social work care.
  • Manage routine and emergency care.
  • Assume basic leadership responsibilities.
  • Provide mentoring of Social Work Case Manager 1's and 2's when requested by the Director of Case Management.
  • Conduct clinical evaluations and assessments.
  • Provide behavioral health assessments and intervention services throughout hospital units, including Bartlett Outpatient Psychiatry Services (BOPS).
  • Develop care plans and discharge planning.
  • Practice professional ethics including the maintenance of appropriate boundaries.
  • Collaborate with partners and maintain frequent communication.
