Baptist Health is seeking a Social Work Case Manager to intervene with patients and families regarding psychosocial needs, assistance, and support in coping with chronic, acute, or terminal illness. Services include consultation, discharge planning, education, counseling, advocacy, and information referrals to address emotional, social, and financial aspects of care. The role involves crisis intervention, coordinating discharge plans, and serving as a liaison between patients and community resources. The Case Manager identifies psycho-social and environmental needs related to admission, diagnosis, treatment, and discharge, working collaboratively with an interdisciplinary team. Responsibilities also include making appropriate referrals to community services, providing inpatient emotional support and counseling, crisis intervention, bereavement services, and assessing/intervening in situations of child or elder abuse/neglect and domestic violence. The position provides care to all patient groups: neonatal, infant, pediatric, adolescent, and adults.
