The Social Work Case Manager is responsible for providing case management services to participants intended to promote self-sufficiency according to the participants’ identified needs and the programs’ focus. The Social Work Case Manager supports participants through a human centered and trauma informed approach, in addition they are engaging, culturally responsive and aware of the diverse needs of various cultures, races, ethnicities and populations. This is a grant-funded position expected to run through November 30, 2028, with potential for extension. The Joint Transitional Housing - Rapid Rehousing program, for youth ages 18-24, will provide up to 24 months of housing/subsidized rental assistance in the location of the youth’s choosing, encouraging youth choice. JTHRRH moves youth forward on the continuum with supportive services and intensive case management in a more independent setting, and social work case managers work with youth to ensure they are able to meet their goals. During JTRRH, youth may receive intensive case management and services to help them to independence and will have access to on-site workshops, counseling, employment support, and programs including: 3E (education, employment, and entrepreneurialism, on-site); Family Functioning (stabilize youth and reunite with family if safe and appropriate, on-site); family finding (connect youth to supports, on-site); mental and behavioral counseling (on-site and off-site); and access to YMCA’s recreational branches (off-site). All youth may be assessed for basic needs and can choose to receive food, clothing, hygiene products, and transportation assistance to move toward independence and self-sufficiency.
