This role involves assisting the team in delivering patient care, considering age and developmental levels, under the direction of professional staff. The Social Work Assistant performs tasks as specified by leadership, maintains an orderly patient flow, and adheres to safety and infection control policies. They also perform related clerical duties to ensure continuity of patient care, including utilizing computer technology, entering data accurately, discharging patients, making referrals, and maintaining a safe environment. Effective communication techniques are essential, demonstrating a professional, caring, and respectful approach with patients, families, and co-workers, while understanding and respecting cultural differences and maintaining confidentiality. The position requires adherence to hospital policies and procedures, participation in staff meetings, and support for the hospital's mission and philosophy. There is an expectation for participation in the development of staff members, meeting annual health assessments, identifying learning needs, and other assigned duties. Specific responsibilities include providing information to outpatient substance programs, offering patients resources such as housing, food, and legal services based on Social Determinants of Health (SDOH), making referrals to community agencies and Health Home, calling patients discharged from the ED, and assisting inpatient Social Work staff with patient resources. Proficiency in EMR (EPIC) for documenting patient care navigator notes and knowledge of email/telephone etiquette are also required.
