Social Work Assistant – Inpatient - Days
About The Position
Social Work Assistants play an intricate role in assisting with patient flow and ensuring quality care and delivery of services for our patients. You’ll partner closely with an interdisciplinary team, working in tandem with the Social Worker and Care Coordination team. Assist in arranging transportation for patients, update and manage documentation within electronic medical records systems. Connect with insurance companies, obtain authorizations and act as liaison with community resources and Nursing Homes. Attend rounds with the physicians and care team and mobilize community agencies and other support networks to give assistance.
Requirements
- Bachelors Degree
- Work experience in a hospital social work department and/or agency
- Proficiency in documentation within electronic information systems
- Bilingual Spanish
