Requirements

  • 1+ years of relevant experience working as a Social Support Worker (SSW)
  • Experience working in healthcare community centers, shelters, or with individual clients
  • SSW Certificate from an accredited educational institution
  • Valid Vulnerable Sector Check < 6 months
  • Standard First Aid CPR (Class C) certificate
  • Covid-19 Vaccination proof (Both Doses)
  • Ability to communicate in English
  • Ability to read medical prescriptions and complete Incident Report Form
  • Ability to accommodate and take On-Call & weekend shifts as required
  • Ability to comply with standard policies/ procedures and ensure safe work practices
  • Eligible Candidates must be flexible in terms of scheduling and have a can-do attitude

Responsibilities

  • Provide emotional, social, and behavioral support to clients in residential or community-based settings
  • Support clients in achieving goals outlined in individualized care or support plans
  • Conduct client check-ins, crisis intervention, conflict resolution, and supportive counseling when required
  • Assist clients in accessing community resources, appointments, housing supports, employment programs, and social services
  • Promote client independence, self-advocacy, life skills development, and community integration
  • Facilitate recreational, educational, and social activities to encourage participation and inclusion
  • Maintain accurate and timely documentation including progress notes, incident reports, behavioral observations, and daily logs
  • Support clients experiencing mental health challenges, addictions, trauma, or dual diagnoses using trauma-informed approaches
  • Ensure the safety, dignity, confidentiality, and well-being of all clients
  • Work collaboratively with case managers, healthcare professionals, families, and community partners
  • Perform light household duties and assist with meal preparation when required within residential settings
  • Adhere to Occupational Health and Safety regulations and agency policies and procedures
  • Perform other duties as assigned
