About The Position

Assist Fellowship Square residents with social service resources, maintain communications with residents and their family members to confirm resident is able to live safely in Independent Living. This position is also responsible for completing the LifeSteps assessment with residents at their annual lease renewal.

Requirements

Minimum of Bachelor's degree in social work preferred.
Minimum of 2 years' experience in a health care facility in Social Services.
Must be able to articulate clearly and precisely.
Must be able to deal effectively with stress created by sick residents, multiple tasks, noises, interruptions, and work cooperatively as part of the health care team while maintaining a pleasant personality.

Responsibilities

Assists in the identification of social, psychological, and emotional strengths and needs of residents.
Services to include placement within the facility or home as required under State and Federal regulations and policies set forth by Christian Care Health Center.
Conduct interviews with family members and, when possible, resident to determine assessment and needs prior to admission.
Schedule admission to facility based upon patient needs and availability.
Develop social history summaries to study, identify and provide for social, emotional and psychological needs of the resident.
Aids in resident's adjustment to their medical and social condition and to our facility.
Works with resident and/or family in planning of future discharge alternatives.
Encourages family contact and support of the resident.
Provides intensive and/or continuing counseling to resident in event of crisis or particular need.
Other duties as assigned.
