About The Position
Baptist Health is seeking a Bachelor's Level Social Services Specialist to join our team. This is a Full-Time Days shift opportunity. The schedule with be M-F 8-hour shifts with no weekends required. The Social Services Specialist will help provide supportive case management, patient and family education, and community resource coordination to improve psychosocial well-being, support treatment adherence, and help patients navigate healthcare needs under physician orders.
Requirements
- 1-2 years Behavioral Health experience
- 1-2 years Case Management experience
- Social Work experience
Responsibilities
- Provide medical social services to patients in the home with Physician's Orders
- Provide rehabilitative and supportive casework to assist patients and their families in adjusting to their health situation
- Responsible for assisting patients and families to understand, accept, and follow medical recommendations as well as identifying appropriate community resources that are available for the patient
