About The Position

The Social Services Manager provides leadership and direct oversight of the Social Services Department at Spectra Health. They are responsible for supervision of Community Health Workers (CHWs), case workers, and social work interns while also being responsible for direct care services that include behavioral health assessments, resource assessments, development of care/service plans, and crisis assessment and intervention. The Social Services Manager works within an integrated team to lead, plan and implement patient programs within Spectra Health.

Requirements

  • Master's Degree in Social Work
  • Credentialed as an LMSW (Licensed Masters Social Worker) in North Dakota
  • Valid Driver's License, Licensed and Insured Vehicle
  • CPR/BLS certification is required or must be obtained within six (6) weeks of employment.

Responsibilities

  • Provide leadership and daily oversight to the Social Services Department within an integrated primary care setting.
  • Represent the Social Services Department on internal committees as assigned.
  • Coordinate and lead efforts related to the execution of the Spectra Plan discount program.
  • Coordinate and lead efforts related to insurance enrollment, especially leading up to Open Enrollment periods.
  • Oversee the hiring/training and provide direct oversight and supervision to case workers, CHWs, and social work interns.
  • Provide brief behavioral health consultation services to patients using the Primary Care Behavioral Health Model as needed.
  • Assist the primary care team in developing processes, behavioral interventions, and patient education to improve self-management of chronic disease.
  • Comply with documentation and billing requirements for services provided in shared electronic health record.
  • Coordinate and provide care that is safe, timely, effective, efficient, and patient centered.
  • Develop care plans and if appropriate, provide case management that includes, but is not limited to, case files, referrals, and other related documents for the treatment of clients; prepare related documentation for civil commitment hearings or other legal proceedings; keep abreast of patient data to ensure appropriate treatment and care are delivered.
  • Develop care plans and work with CHW staff to implement and manage patient care plans.
  • Initiate and manage assessment and referrals to CHW staff.
  • If appropriate, provide Free Through Recovery/Community Connect Care Coordination services to referred patients.
  • Handle case assignments, draft service plans, review case progress, and determine case closure in compliance with program guidelines.
  • Coordinate with providers, staff and other agencies to deliver integrated patient care.
  • Ability to understand, navigate, and communicate billing and insurance complexities to patients.
  • Strong working knowledge of the Spectra Plan discount program.
  • Willingness to accompany patients to appointments within Spectra Health and to partner agencies.
  • Manage data collection requirements.
  • Develop and maintain relationships with other social services agencies.
  • Assist or conduct outreach efforts on behalf of Spectra Health.
  • Assist with grant writing and ensure execution and compliance of grant requirements.
  • Coordinate with certified interpreter staff to meet patient needs.
  • Other duties as deemed necessary by the Director of Community Engagement and Innovation.
