About The Position

This position is for a Social Worker specializing in Pre/Post Liver Transplant Patients. The role involves managing a caseload that varies, with approximately 10 new patients per week. Responsibilities include verifying caregivers over the phone and completing psychosocial evaluations for both pre- and post-liver transplant patients. While primarily stationed outpatient, the role may require providing evaluations for inpatient cases based on departmental needs. Perm candidates from other providence facilities (non-Swedish) are eligible for a travel contract if they have been separated for at least 3 months. All Swedish facilities honor active compact licenses.

Requirements

• Licensed Independent Clinical Social Worker (LICSW)
• Active compact license (honored by all Swedish facilities)
• Ability to manage a variable caseload of approximately 10 new patients per week.
• Experience with psychosocial evaluations for transplant patients.

Nice To Haves
