About The Position
Diablo Valley Post Acute is seeking a compassionate, organized, and dedicated Social Services Director to join our team. This is an excellent opportunity for a professional who is passionate about advocating for patients and supporting residents and their families throughout their healthcare journey.
Requirements
- Experience in social services within a skilled nursing, post-acute, or healthcare setting preferred.
- Knowledge of discharge planning, resident rights, and regulatory requirements.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work collaboratively with residents, families, physicians, and healthcare professionals.
Responsibilities
- Assess residents' psychosocial needs and develop individualized care plans.
- Coordinate discharge planning and transitions of care.
- Provide emotional support, counseling, and advocacy for residents and families.
- Collaborate with the interdisciplinary team to ensure quality resident care.
- Connect residents and families with appropriate community resources and services.
- Maintain accurate documentation and ensure compliance with state and federal regulations.
- Participate in care conferences and quality improvement initiatives.
