The Social Services Director is responsible for handling pressure, maintaining objectivity, confidentiality, organization, planning, reporting skills, persistence, and being proactive. This role requires strong listening and verbal communication skills, and the ability to build relationships with residents and their families. The primary goal is to assist residents in achieving the highest practicable level of self-care, independence, and well-being by providing medically related social services. This involves evaluating social and family information, addressing psychological and emotional needs, developing care plans, and providing emotional support. The role also includes assisting residents with health care decisions, personal shopping, inventorying belongings, coordinating responses to missing items, and securing community resources. A key aspect is developing and maintaining strong working relationships with other departments and outside agencies. The position involves documenting resident social service status, assisting with appointments and ancillary patient services, and coordinating discharge planning with relevant parties. Additionally, the Social Services Director will provide in-service training to staff and counsel residents and family members. This is a full-time position.
