Bear Creek Nursing & Rehab Center is seeking an experienced Social Services Director (BSW required) to lead and oversee our social services program. This is a key leadership role where your voice directly impacts resident experience, care transitions, and overall quality of life.

Requirements

  • Bachelor of Social Work (BSW) required
  • Skilled Nursing Facility (SNF) / Long-Term Care (LTC) experience required
  • Strong knowledge of: Psychosocial assessments & care planning
  • Strong knowledge of: Discharge planning & care transitions
  • Strong knowledge of: Regulatory compliance in long-term care
  • Excellent communication, documentation, and interpersonal skills
  • Ability to build trust with residents, families, and team members
  • Strong organization and time management

Responsibilities

  • Lead and manage the facility’s social services program
  • Conduct psychosocial assessments, care planning, and documentation
  • Serve as a primary advocate for resident rights, dignity, and emotional well-being
  • Guide residents and families through admissions, care transitions, and discharge planning
  • Coordinate and lead care conferences and behavioral interventions
  • Collaborate with the interdisciplinary team (nursing, therapy, leadership)
  • Assist families with resources, financial processes, and long-term planning
  • Ensure compliance with federal and state regulations for social services in SNF
  • Provide staff education on communication, behaviors, and resident-centered care
