River Oaks of Minnesota is seeking a full-time Social Services Director to support the psychosocial, emotional, and behavioral health needs of residents within our assisted living community. Requirements
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred
- Experience working with individuals with mental illness, behavioral health needs, or in assisted living/long-term care settings strongly preferred
- Strong understanding of mental health diagnoses, symptoms, and interventions
- Ability to de-escalate crisis situations and manage challenging behaviors
- Excellent communication, organization, and interpersonal skills
- Ability to work collaboratively with multidisciplinary teams
Responsibilities
- Conducting psychosocial assessments to evaluate resident emotional and social needs
- Providing emotional support, active listening, and crisis intervention when needed
- Advocating for resident rights, preferences, and person-centered care
- Developing and updating individualized service plans focused on mental health and social needs
- Coordinating with families, guardians, mental health providers, and outside agencies
- Monitoring changes in resident mood, behavior, and functioning
- Supporting behavioral interventions and care planning initiatives
- Assisting with admissions, transitions, and discharge planning to ensure continuity of care
- Maintaining accurate documentation in compliance with facility policies and state regulations
