About The Position

Napa Post Acute is seeking an experienced, compassionate, and highly organized Social Services Director to lead our Social Services department. This is an excellent opportunity to join a dedicated interdisciplinary team committed to delivering exceptional care and support to our residents and their families. As our Social Services Director, you will play a critical leadership role in enhancing residents' quality of life, supporting successful care transitions, and ensuring compliance with all state and federal regulations.

Requirements

  • Bachelor's Degree in Social Work (BSW) or Bachelor's Degree in a related human services field such as Sociology, Psychology, Rehabilitation Counseling, or Gerontology
  • Experience in healthcare social services, skilled nursing, long-term care, hospital, hospice, or related healthcare setting
  • Must meet all applicable California state and federal qualifications for social services personnel in skilled nursing facilities
  • Must meet California Title 22 Social Services Director requirements for a 120-bed skilled nursing facility
  • Strong knowledge of discharge planning, care coordination, and resident advocacy
  • Excellent communication and organizational skills

Nice To Haves

  • Skilled Nursing Facility (SNF) experience
  • Previous leadership or supervisory experience in social services
  • California social work licensure, when applicable

Responsibilities

  • Direct and oversee all Social Services functions within the facility
  • Complete psychosocial assessments and care plans
  • Coordinate discharge planning and community resource referrals
  • Assist residents and families with adjustment, coping, and crisis intervention needs
  • Facilitate resident and family meetings
  • Collaborate with nursing, therapy, physicians, and interdisciplinary team members
  • Ensure compliance with federal regulations, California Title 22 requirements, and facility policies
  • Maintain comprehensive social services documentation
  • Support resident rights, advocacy, and quality of life initiatives
  • Develop and maintain relationships with community agencies and healthcare partners
