About The Position

The Social Service Director is responsible for the provision of resident services to improve and/or maintain their psychosocial well-being. This role involves frequent rounding to visit residents, assessing psychosocial needs, and reviewing nursing reports for potential intervention. The Social Service Director also manages referrals to psychosocial clinicians, seeks resources from outside providers, and maintains positive relationships with case managers and providers. Additionally, this role involves obtaining and reviewing resident PASRR information, potentially auditing and submitting renewals for 142s, and assisting with admission paperwork. The Social Service Director oversees psychiatric, dental, and vision services, completes MDS assessments, assists residents with goals and needs, and ensures proper discharge planning. This position is a key member of the Interdisciplinary Team (IDT Team) and participates in various meetings and quality assurance programs. The Social Service Director acts as a resident advocate, educates staff on Resident's Rights, and makes introductory visits with new residents and employees. The role requires professional representation of the facility, effective communication, and strict adherence to confidentiality and HIPAA regulations. Compliance with Legacy's Compliance program, Code of Conduct, and Dress Code is mandatory. Duties are subject to change based on facility needs.

Requirements

  • Not found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting.
  • Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field (e.g., sociology, gerontology, special education, rehabilitation counseling, psychology).
  • One year of supervised social work experience in a health care setting working directly with individuals.

Nice To Haves

  • Experience in a Long-Term care setting preferred but not required.
  • For facilities with over 120 beds and/or with respiratory units, MSW may be required.

Responsibilities

  • Provide resident services to improve and/or maintain their psychosocial well-being.
  • Frequently round the building to visit with residents to assess psychosocial needs.
  • Review 24-hour nursing reports to identify documentation suggesting the need for psychosocial intervention.
  • Maintain a list of residents requiring psychosocial clinician visits.
  • Seek resources for resident intervention from outside providers when needed.
  • Maintain positive work relationships with case managers and providers.
  • Obtain, review, and keep copies of resident Level II PASRR and make referrals as appropriate/recommended by the Office of Behavioral Health (OBH).
  • Audit and submit renewals for 142s.
  • Assist with completion of admission paperwork.
  • Oversee psychiatric, dental, and vision services.
  • Complete MDS assessments timely and accurately.
  • Assist residents with goals and needs.
  • Ensure proper discharge planning for each resident and document such.
  • Contribute to the resident care plan.
  • Participate in Daily QA (morning) Meetings, Monthly/Quarterly QA, Safety Committee Meetings, Resident Council Meetings when requested, Care Plan Meetings, in-service trainings, continuing education programs, and all other meetings called by Legacy or licensing entities.
  • Serve as a member of the Interdisciplinary Team (IDT Team).
  • Be available during an emergency to assist the facility and assess residents as needed.
  • Assist in maintaining quality resident care through the establishment of quality assurance/performance improvement monitoring programs (QAPI) and standards.
  • Implement measures to meet regulatory and legal requirements.
  • Participate in community events and marketing plans and programs as needed.
  • Act as a resident advocate and educate staff on Resident's Rights.
  • Make introductory visits with new residents and employees timely.
  • Attend new hire orientation program when requested to greet and brief new employees on the facility mission and the Social Service Director's role.
  • Participate in the Ambassador Rounds Program.
  • Act in ways that promote professional and positive representation of the facility to the community.
  • Communicate in a professional and courteous manner using effective verbal and non-verbal communication skills with residents, their families, co-workers, and vendors.
  • Maintain confidentiality of department, resident, employee information, communication, and documentation, as well as compliance with HIPAA regulations.
  • Clock in and out for all working/training hours; working off the clock is not permitted.
  • Maintain compliance with Legacy's Compliance program and plan.
  • Adhere to Legacy's Code of Conduct and Dress Code.
