About The Position

The Social Services Director will manage the Social Services department and staff, which is a small department with less than 10 staff. This role involves initiating treatment plans, completing psychosocial assessments, facilitating group sessions, and developing discharge plans. The Director will also make appropriate referrals to community resources and various treatment programs, and conduct discharge meetings with patients and their families.

Requirements

  • Licensed Clinical Social Worker (LCSW) / Licensed Clinical Professional Counselor (LCPC).
  • Experience supervising staff.
  • Successful completion of background check.
  • May require working weekends and holidays.

Responsibilities

  • Initiate treatment plan, complete social services portion of treatment plan (within 24 hours).
  • Review patient progress in treatment plan, complete psychosocial assessments on each patient (within 72 hours).
  • Facilitate social services group on scheduled days.
  • Complete a tentative discharge plan.
  • Discharge meeting with patient, and family (if requested), to discuss the discharge and crisis plan as well as patient and family expectations.
  • Make appropriate referrals to community resources and scheduling follow-up appointments.
  • Refer patients to in-pt. or out-pt. substance abuse treatment programs, partial hospitalization programs, counseling, PCP, med management, CBRS, case management, SUD treatment, PCS services, and home health, etc.
  • Manage the Social Services department and staff (small department, <10 staff).
