The Social Services Director is responsible for handling pressure, objectivity, confidentiality, organization, planning, reporting skills, persistence, proactivity, listening, verbal communication, and maintaining positive relationships with residents and their families. This role involves establishing courses of action by exploring options and setting goals with residents and their families, assisting them in achieving the highest practicable level of self-care, independence, and well-being. The position provides medically related social services to ensure the highest practicable physical, mental, and psychosocial well-being of each resident is attained or maintained. Key duties include evaluating social and family information, assessing psychological and emotional needs, developing care plans, providing emotional support, addressing emotional problems, assisting residents and families with coping mechanisms for physical disabilities, health-related fears, and grief, and aiding with healthcare decisions, personal shopping, and inventory of patient belongings. The role also coordinates responses to reports of missing, lost, or stolen belongings, secures community resources, and assists in obtaining appropriate prosthetics and assistive devices. A strong working relationship with other departments and outside community agencies is essential for meeting resident needs. Documentation of resident social service status, scheduling of ancillary patient services, and assisting in discharge planning with relevant agencies and the interdisciplinary team are also core functions. The position may also involve providing in-service training to staff and counseling residents and family members.
