About The Position

The Director of Social Services is responsible for identifying the medically related social and emotional needs of patients/residents. This role involves meeting those needs through individual and group social services programming, promoting patients'/residents' potential during their stay, and collaborating with physicians and patients in preparation for discharge.

Requirements

Minimum of a Bachelors degree in social work or in human services fields.
Licensed per the state requirements or eligible for licensure.
Skills in communication, documentation, assessment, and social work methods and techniques.
At least one year of experience in a long-term care setting and/or related healthcare setting.
Ability to read, write, and speak the English language.

Responsibilities

Develops social histories of patients/residents and creates a social assessment and care plan for each patient/resident that identifies medically related social and emotional problems and needs.
Participates in interdisciplinary care plan conferences and contributes to the development of care plans for patients/residents.
Establishes specific social action plans and realistic goals for each patient consistent with medically related social and emotional needs.
Designs and develops social service programs that meet medically related social and emotional needs of patients/residents and comply with regulatory guidelines.
Interprets and promotes patients'/residents' rights and the "Patients' Bill of Rights" with the Administrator.
Provides individual and group help for patients/residents and families during times of adjustment, crisis, or need.
Orients patients/residents and families to the facility environment and assists their adjustment through individual and group programs.
Completes the Social Services section (sections E & F) of the MDS.
Assists with the implementation of all RCMS and #13 Advanced Directives.
May coordinate the admissions process for new or returning patients.
Coordinates discharge planning with staff, patients/residents, and families.
Coordinates social services "in-service" and educational programs for staff.
Completes all necessary and supportive documentation, including progress notes, intervention notes, and adjustment notes, in accordance with state and federal regulations.
May identify and promote a family council committee.
Functions as a Team Leader/Role Model.
Serves on various committees as requested by the Administrator.

Responsible for assuring patient/resident safety.
