About The Position

In the Social Service Director role, you will be responsible for overseeing the development, implementation, supervision and ongoing evaluation of the Social Services Department designed to meet and assist residents in attaining or maintaining their highest practicable well-being. Journey Healthcare is seeking a passionate individual to lead their Social Services Department, contributing to exceptional quality outcomes and a world-class Care Team. This role is crucial in supporting residents' well-being and ensuring compliance with state and federal regulations.

Requirements

A minimum of a bachelor’s degree in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology.
One year of supervised social work experience in a health care setting working directly with individuals.
License Social Worker License in required state.

Nice To Haves

The regulations do not require a Social Worker when a facility has equal to or less than 120 beds.

Responsibilities

Overseeing the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
Identifying the need for medically related social services and ensuring their provision in accordance with State and Federal regulations.
Completing and/or directing/delegating the completion of the social services component of the comprehensive assessment.
Contributing to and/or directing/delegating contribution of social services goals and approaches to the care plan.
Overseeing preadmission services for inbound residents.
Facilitating residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
Overseeing the establishment of departmental QA procedures and modification of those procedures where appropriate.
