Requirements

  • Bachelor's Degree in Social Work or Human Services required
  • Minimum of two years of supervised social work experience in a healthcare setting
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Ability to maintain confidentiality and professionalism
  • Experience working with residents, families, and interdisciplinary teams
  • Strong problem-solving and conflict-resolution skills
  • Ability to work independently while managing multiple priorities

Nice To Haves

  • MSW preferred
  • Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred

Responsibilities

  • Assess residents' social, emotional, family, and environmental needs
  • Develop and implement individualized social service care plans
  • Assist residents in achieving the highest practicable level of independence and well-being
  • Provide emotional support and counseling to residents and family members
  • Assist residents and families with healthcare decisions and care transitions
  • Coordinate discharge planning with community agencies, healthcare providers, and family members
  • Arrange ancillary services including optometry, podiatry, dental, and psychiatric appointments
  • Connect residents and families with community resources and support services
  • Assist with inventory and tracking of resident belongings
  • Respond to concerns regarding missing, lost, or stolen belongings
  • Maintain accurate and timely documentation of social service interventions
  • Participate in care plan meetings and interdisciplinary team discussions
  • Develop strong relationships with residents, families, staff, and community partners
  • Provide in-service education and training as needed
