Social Services Director – Full-Time $50,000–$60,000/year Greer, SC About Us Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. Requirements
- Bachelor's Degree in Social Work or Human Services required
- Minimum of two years of supervised social work experience in a healthcare setting
- Excellent communication and interpersonal skills
- Strong organizational and time-management abilities
- Ability to maintain confidentiality and professionalism
- Experience working with residents, families, and interdisciplinary teams
- Strong problem-solving and conflict-resolution skills
- Ability to work independently while managing multiple priorities
Nice To Haves
- MSW preferred
- Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred

Responsibilities
- Assess residents' social, emotional, family, and environmental needs
- Develop and implement individualized social service care plans
- Assist residents in achieving the highest practicable level of independence and well-being
- Provide emotional support and counseling to residents and family members
- Assist residents and families with healthcare decisions and care transitions
- Coordinate discharge planning with community agencies, healthcare providers, and family members
- Arrange ancillary services including optometry, podiatry, dental, and psychiatric appointments
- Connect residents and families with community resources and support services
- Assist with inventory and tracking of resident belongings
- Respond to concerns regarding missing, lost, or stolen belongings
- Maintain accurate and timely documentation of social service interventions
- Participate in care plan meetings and interdisciplinary team discussions
- Develop strong relationships with residents, families, staff, and community partners
- Provide in-service education and training as needed
