About The Position

Providence Place, a Lifespark Skilled Nursing Community, is looking for a dedicated and passionate Social Services Director, Licensure Required to join their growing team. They have openings on their TCU and LTC units and offer in-house dialysis services. Providence Place is conveniently located on the bus route in southern Minneapolis. They provide services for seniors in a TCU (Transitional Care Unit) Care, Long-Term Care, and Memory Care. They have been helping seniors Age Magnificently for over 25 years and are part of Senior Care Communities, a local non-profit supporting seniors. The Director of Social Services becomes family and impacts the lives of residents in a Lifespark Senior Living Community. In this position, the individual will be responsible for the operations of the Social Services department, providing for each resident’s social, emotional, and psychological needs and the continuing development of the resident’s full potential from admission to discharge.

Requirements

  • Valid state licensure as a Social Worker.
  • Post Secondary Education with a bachelor’s degree in social work or human services field including but not limited to Sociology, Psychology, Special Education, or Rehabilitation Counseling.
  • A minimum of two years in long-term care or acute care is preferred.
  • One year of supervised social work experience in a health care setting working directly with individuals.

Responsibilities

  • Provide oversight and supervision of all functions and staff of the Social Service department.
  • Lead and participate in essential training of staff.
  • Review and adhere to departmental budget.
  • Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident.
  • Participate in the pre-admission screening of all potential residents and provide information to residents and families during admission.
  • Orient the resident to the long-term care environment and facilitate adjustment upon placement.
  • Document progress notes which relate to each resident’s care plan when necessary.
  • Assist the resident and resident’s family in discharge and placement planning.
  • Organize family groups to promote communications, education and support between family members, facility staff, and administration and provide counseling, as needed.
  • Assist in the education of the community regarding aging, resident rights, facility services and other related topics.
  • Participate in daily Interdisciplinary meetings and other community meetings including Care Plan meetings and Quality Improvement.
  • Must be adaptable, flexible to changes, and able to prioritize and organize work efficiently to accomplish workload within time allotted.
  • Maintain a positive attitude in relation to customer service program.
  • Comply with established safety rules and regulations.
  • Promptly reports accidents, incidents, and unsafe and hazardous conditions/equipment to supervisor.
  • Understand, comply with, and promote all rules regarding resident’s rights.
