About The Position

The Social Services Designee is responsible for planning, implementing, & evaluating the social services program for residents of The Chateau. He/she will follow & establish policies, procedures & professional standards to ensure the emotional & social needs of the residents are met on an individual basis & meet or exceed all State & Federal guidelines.

Requirements

  • Excellent oral and written communication skills in the English language required
  • During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
  • Graduate of an accredited 4-year college of university with a major in Social Work or healthcare related field.
  • Previous social service or case management experience in a healthcare setting.
  • Understands Federal & Medicare rules & regulations.

Responsibilities

  • Visits residents to assess social service’s needs, ensure quality care and reduce negative outcomes.
  • Coordinates the discharge planning process among residents, family, staff & outside community agencies in order to assure continuity of care &/or services.
  • Responds to resident & family concerns, arranges care conferences, files grievances.
  • Utilizes problem-solving skills to address concerns & inquiries in an open fashion promoting positive relations with residents & families.
  • Informs residents & their families about their various rights & responsibilities upon admission, i.e., the Medicare program, advance directives, personal & property rights, etc.
  • Facilitates open & direct communication between residents & staff.
  • Maintains a good working relationship with interdepartmental personnel, community health professionals, & social service agencies to assure all residents' social service needs are met.
  • Attends & participates in regularly scheduled meetings, i.e., care plan, resident & family council, continuous quality improvement & staff meetings, lie care meetings.
  • Completes required documentation in a timely manner i.e., resident assessment, quarterly progress notes, care Ian, MDS, & quality assurance studies.
  • Coordinates, investigates, & reviews complaints, or grievances & submits written reports to the Associate Administrator
  • Investigates & reports all allegations of abuse, neglect, or exploitation as necessary.
  • Review code status with resident/healthcare surrogate/medical power of attorney quarterly for all long-term care residents.
  • Follow up with family/responsible party upon resident expiration.
